Abstract

Every technological and scientific development in Endodontology aims to preserve the tooth in the dental arch. New evaluation methods and treatment techniques can be the cause for controversies, with the consequence that it is not always possible to choose a commonly accepted treatment plan. The dilemma in decision making and the difficulties in treatment and restorative management become greater in teeth with a doubtful prognosis due to pathology and marginal tooth substance, especially in cases where endodontic treatment has been previously performed. Surgical endodontics performed with modern microsurgical techniques leads to predictable therapeutic results, even in posterior teeth, when case selection is done with strict criteria and all etiological and prognostic factors are carefully evaluated.

Typical cases that we deal with on a daily basis, sometimes "borderline " and sometimes " heroic ", will be discussed interactively, with the aim of raising clinical concerns regarding tooth preservation, functional restoration and viability. Can we finally predict the unpredictable? How can such a question be answered when the prognosis of an endodontically treated tooth is characterized by words such as: doubtful, unpredictable, unfavorable, short-term, poor? It is then that the daily stories of endodontic "madness " begin...
