Dear Colleagues,
Abstract
The restoration of endodontically treated teeth constitutes a challenge for the clinician in the daily dental practice.
Due to extended decay, trauma or previous restorations root canal treated teeth present excessive loss of tooth structure. Furthermore, the various instruments and materials used for the preparation and obturation of the root canals system may produce significant changes in the physical properties of dentin (its micro-hardness and modulus of elasticity) as well as loss of the proprioception of the tooth. Consequently, root canal treated teeth are in greater danger compared to vital teeth.
The restoration of an endodontically treated tooth is imperative. However, there is confusion regarding the treatment options available within the literature.
The fundamental criteria for choosing the type of restoration are reliant on the height and thickness of the axial walls of the tooth, its position in the dentition, its functional requirements and finally, esthetics. The decision on whether or not a post and core shall be placed depends on the amount of the remaining tooth structure. Greater attention was paid in the past to the necessity of internal reinforcement of root canal treated teeth in order to increase the retention and stability of the prosthetic restoration. However, recent data reveal that posts neither reinforce internally a tooth nor decrease the possibility of tooth fracture. On the contrary, posts increase the retention of the restoration.
In the seminar, we will analyze in detail all the criteria that need to be taken into consideration for the restoration of an endodontically treated tooth. Moreover, we will focus on the therapeutic options, protocols and materials available in the prosthetic field in order to develop a personalized treatment plan adapted to the needs of each tooth with the objective to retain as much healthy tooth substance as possible along with following the basic principles of biomechanics.
